	Application: New procedure as of June 1, 2010 Please note that as of June 1, 2010, the DS-156 application form for a visa to the United States will no longer be used. It will be replaced by the new DS-160 visa form. (see "download" column) The new form will be used to collect all necessary information, including the data that was previously requested on forms DS-157 and DS-158, which will be eliminated. Each application form must be accompanied by a photograph of the applicant (5x5 cm, with a white background and well-defined contrast), without which the application procedure cannot proceed.
